  A 64 year old woman, who is a farmer by occupation, presented to the hospital with- • dry cough since 6 years • SOB since 3 days • decreased urinary output since 3 days. Date of admission - 2pm, 16/08/21. HISTORY OF PRESENT ILLNESS • PT was apparently a symptomatic 6 years back. • PT started getting intermittent cough since then.  • She wrongly self-diagnosed herself with Tuberculosis and started using her husband's TB medication on SOS basis.  • The patient also took medication to manage pain on SOS basis.  • Her intermittent cough turned persistent over the last 3 months and got admitted to a hospital ( in their mandal) suspecting it was Covid19. • The hospital later diagnosed her with kidney failure and DM. • The doctor who prescribed the medications for her kidney failure 3 months back, was not aware of the other medications she was taking (TB medications). • The patient joined the current hospital on the basis of referral.  • 3 days back, the pati...

A 65 year old woman with vomiting, stomach ache since 20 days.

A 65 year old woman presented to the hospital with the complaints of- • vomiting • stomach ache -since 20 days HISTORY OF PRESENT ILLNESS. A 65 year old married woman and a mother of 4, who was an occasional alcoholic, who was a homemaker, was apparently asymptomatic before 20 days. Her daily routine included waking up at 6am, cooking food for her family, separating rice grains from rice chaff, having food and sleeping in the afternoons. Whenever she could afford, she used have alcohol. 20 days back, there was a suddenly onset of stomach pain and vomiting. The vomiting was not projectile in nature, based on the information provided by the patient. This caused her great distress and considerably decreased her appetite. She went to a hospital in Nalgonda few days back. She was referred to Kamineni and has been admitted here for 11 days. A 60 year old man with pedal oedema since 15 days, dypnea since 5 days, fever since 5 days.

 A 60 year old man, who worked as a lorry driver, presented to the hospital with- • Pedal oedema of since 15 days, • Dyspnea since 5 days, • Fever since 5 days, • Decreased urine output since 3 days, Date of admission - 2/2/22 HISTORY OF PRESENT ILLNESS • Patient was apparently asymptomatic 15 days back. • Patient wakes up at 6am every morning. He has his lunch at 2pm. He sleeps in the afternoon. He has dinner at 9pm and sleeps by 10pm. •Patient is an ex lorry driver and lives a very sedentary life.  • 15 days ago, they developed pedal oedema of pitting type — which extended upto ankles. • They also presented with fever, which was continuous since 5 days. • Patient also presented with grade 4 shortness of breath since 5 days. • Vomiting of one episode which was non projectile 5 days back. • Patient also suffered with uncontrolled hemoptysis since 5 days back.  • 3 days ago, when the pt. was undergoing dialysis, the patient suffered from a cerebro vascular accident.  ...
